How do I get started? 


• Please complete the contact us form on our website to schedule a session
• Once appointment is scheduled, intake forms will be sent and need to be completed at least 24 hours before your first session
• Frequency of sessions will be discussed during your initial session

How long are my sessions? 


• Intake sessions will be 60 minutes • Follow up therapy sessions will be 53 minutes

Do you accept insurance?

• We are in network with BCBS PPO and Blue Choice Plans
• If you are using an out-of-network insurance plan, you will be responsible to pay for the full cost of the session at the self-pay rate. A superbill can be provided to submit to your insurance company for reimbursement.
• For self-pay rates, please contact the office to discuss costs and options

• Payment is required at the time of services • You are responsible for all deductibles and copays/coinsurance specific to your health insurance plan

What is your cancellation policy?


• Appointments canceled with less than 24 hour notice will be subject to a cancellation fee
